We are writing to inform you of administrative changes that will not affect your medical records and is intended to ensure uninterrupted access to your medical care.

RSV Medical, PLLC is transitioning to a new administrative services provider as of December 4, 2025.

Transcend Company, Inc. (“Transcend”) is a management platform that provides administrative and operational support to independent healthcare providers. Transcend does not provide medical or pharmacy services, employ healthcare providers, or influence clinical decisions.

Transcend is transitioning to different medical services provider with an affiliated physician practice. These changes relate to administrative support only and do not impact the clinical care you receive through RSV Medical or your rights as a patient.

Your Care Options

Option 1: Continue Care with Dr. Veyna and the RSV Medical Clinical Team

You may elect to continue receiving care from Dr. Richard Veyna and the RSV Medical clinical team. Under this option, Dr. Veyna and RSV Medical will continue to provide all medical services, including evaluations, prescriptions, treatment, and follow-up care, supported by a new administrative services provider. No further action is required to remain with your current provider, Dr. Richard Veyna and RSV Medical.

Option 2: Start Care with Dr. Kurip 1977 MESK PLLC

Start Care with Dr. Sunil Kurip and 1977 Mesk PLLC by electing to continue to receive administrative support through Transcend through its new affiliated medical provider, 1977 MESK PLLC and Dr. Sunil Kurip, who will provide all medical services, including evaluations, prescriptions, treatment, and follow-up care. If you select this option, you will establish yourself as a patient with the new provider and complete the forms necessary to transition to this new medical team.
Option 3: Transition Care to a New Provider
You may choose to transition your care to a different healthcare provider of your choosing. In accordance with HIPAA, you have the right to obtain a copy of your medical records or to request that your records be transferred to a provider you designate.

Medical Records and Privacy

There are no changes to your medical records as a result of this administrative transition.

Dr. Veyna and RSV Medical, PLLC, as your medical provider, will remain custodians of your Protected Health Information (“PHI”) as your medical provider. Transcend as the former administrative entity for Dr. Veyna and RSV Medical, PLLC, will continue to maintain your PHI in accordance with applicable law. In either case, your medical records, medical history, and treatment information will continue to be securely maintained in compliance with HIPAA and all applicable federal and state privacy laws.

Important Information About Your Choice

If you elect to continue to receive care with Dr. Veyna and RSV Medical, PLLC under Option 1, no further action is necessary. If you elect care with 1977 MESK PLLC and Dr. Sunil Kurip through Transcend Company, Inc, under Option 2, you will need to complete the form necessary to transfer your medical care to this new provider.
