Change of Provider Letter
Important Notice Regarding Administrative Services
Dear Patient,
We are writing to inform you of administrative changes that will not affect your medical records and are intended to ensure uninterrupted access to your medical care.
RSV Medical, PLLC is transitioning to a new administrative services provider as of December 4, 2025.
Transcend Company, Inc. (“Transcend”) is a management platform that provides administrative and operational support to independent healthcare providers. Transcend does not provide medical or pharmacy services, employ healthcare providers, or influence clinical decisions.
Transcend supports more than one affiliated, independent medical group, and patients may be offered care through any of those affiliated medical groups based on the requested service line and state of licensure. These changes relate to administrative support only and do not impact the clinical care you receive through RSV Medical or your rights as a patient.
Your Care Options
Option 1: Continue Care with Dr. Veyna and the RSV Medical Clinical Team
You may elect to continue receiving care from Dr. Richard Veyna and the RSV Medical clinical team. Under this option, Dr. Veyna and RSV Medical will continue to provide all medical services, including evaluations, prescriptions, treatment, and follow-up care, supported by a new administrative services provider. No further action is required to remain with your current provider, Dr. Richard Veyna and RSV Medical.
Option 2: Start Care with a Transcend-Affiliated Medical Group (1977 MESK PLLC or euVita Medical Group PLLC)
You may elect to continue to receive administrative support through Transcend by establishing care with one of Transcend’s affiliated, independent medical groups. As of the date of this letter, those medical groups are 1977 MESK PLLC (Sunil Kurup, M.D.) and euVita Medical Group PLLC (Kenneth Rice, M.D.). The medical group with which you will establish care will be determined by the service line you request and the state in which you reside (subject to clinician licensure), and will be communicated to you before any clinical evaluation. The selected medical group will provide all medical services, including evaluations, prescriptions, treatment, and follow-up care. If you select this option, you will establish yourself as a patient with the new medical group and complete the forms necessary to transition to this new medical team.
Option 3: Transition Care to a New Provider
You may choose to transition your care to a different healthcare provider of your choosing. In accordance with HIPAA, you have the right to obtain a copy of your medical records or to request that your records be transferred to a provider you designate.
Medical Records and Privacy
There are no changes to your medical records as a result of this administrative transition.
Dr. Veyna and RSV Medical, PLLC, as your medical provider, will remain custodians of your Protected Health Information (“PHI”) as your medical provider. Transcend, as the former administrative entity for Dr. Veyna and RSV Medical, PLLC, will continue to maintain your PHI in accordance with applicable law. If you elect Option 2, the Transcend-affiliated medical group with which you establish care (1977 MESK PLLC or euVita Medical Group PLLC, as applicable) will become the custodian of any new PHI generated through that medical group, and any sharing of PHI between affiliated medical groups will be governed by the joint Notice of Privacy Practices posted on the Transcend Company, Inc. website. In either case, your medical records, medical history, and treatment information will continue to be securely maintained in compliance with HIPAA and all applicable federal and state privacy laws.
Important Information About Your Choice
If you elect to continue to receive care with Dr. Veyna and RSV Medical, PLLC under Option 1, no further action is necessary. If you elect care with a Transcend-affiliated medical group through Transcend Company, Inc. under Option 2, you will need to complete the form necessary to transfer your medical care to the assigned medical group.
