**This electronic gift card (the “Card”) is subject to the following terms and conditions: (1) Non-Refundable: The Card is non-refundable and cannot be redeemed for cash or cash equivalents. Any unused balance on the Card will not be refunded or returned to the cardholder under any circumstances. (2) No Expiration Date: The Card does not expire, providing the cardholder with flexibility and peace of mind. (3) Lost or Stolen Cards: The issuer is not responsible for lost, stolen, or damaged Cards. It is the cardholder's responsibility to safeguard the Card and its details. Any lost or stolen Cards will not be replaced. (4) Redemption: The Card can be redeemed for eligible purchases through Transcend Company. (5) Balance Inquiries: The cardholder may check the balance on the Card by consulting their Wellness Specialist or by contacting the Transcend office at (248) 520-5407. (6) Use of Multiple Cards: Only one gift card can be utilized per transaction, in accordance with the issuer's policy. (7) Compliance with Law: Use of the Card is subject to all applicable laws and regulations, including any restrictions on the issuance, use, and redemption of gift cards. By purchasing, using, or accepting the Card, the cardholder agrees to abide by these terms and conditions.
