Electronic Gift Card Disclaimer

This electronic gift card (the “Card”) is subject to the following terms and conditions:

  • Non-Refundable: The Card is non-refundable and cannot be redeemed for cash or cash equivalents. Any unused balance on the Card will not be refunded or returned to the cardholder under any circumstances.
  • No Expiration Date: The Card does not expire, providing the cardholder with flexibility and peace of mind.
  • Lost or Stolen Cards: The issuer is not responsible for lost, stolen, or damaged Cards. It is the cardholder's responsibility to safeguard the Card and its details. Any lost or stolen Cards will not be replaced.
  • Redemption: The Card can be redeemed for eligible purchases through Transcend Company.
  • Balance Inquiries: The cardholder may check the balance on the Card by consulting their wellness specialist or by contacting the Transcend office at (248) 520-5407.
  • Use of Multiple Cards: Only one gift card can be utilized per transaction, in accordance with the issuer's policy.
  • Compliance with Law: Use of the Card is subject to all applicable laws and regulations, including any restrictions on the issuance, use, and redemption of gift cards.

By purchasing, using, or accepting the Card, the cardholder agrees to abide by these terms and conditions. The issuer reserves the right to modify or terminate the Card program or these terms at its discretion.

Patients who are found to have submitted fraudulent information will be terminated from any health program offered by Transcend. Any medication prescribed is only for the use of the patient and is not to be transferred, distributed, modified, or used by any other person(s). Transcend Company Inc complies with HIPAA and wants to exchange text messages with you. By clicking Submit below, you agree to receive text messages from Transcend Company. These text messages may contain details about your care providers, programs, healthcare reminders, or conditions (e.g. low testosterone, vitamin D deficiency, low blood sugar, etc.). You acknowledge and agree that text messages are never completely private or secure. You understand text messages will be sent unencrypted and may be read or intercepted by others.
